Introduction and General Information

The medical record serves to document all that happened during the course of a patient encounter, including history (subjective data; i.e., what the patient told you), examination (objective data; i.e., non-historical data that you gathered during the course of the examination), and conclusions that should be justified by the subjective and objective data that you gather during the encounter.

This document will describe my expectations for you in clinc.  Please review it in advance of clinic.
