36 Quick Instructions – iCare Rebound Tonometry
Jeffrey D. Perotti, M.S., O.D.
Introduction
The iCare tonometer is a safe, reliable alternative to Goldmann tonometry, allowing you to safely and accurately measure intra-ocular pressure without the use of an ocular anesthetic
Indications
- Patient is unable to perform Goldmann tonometry.
- Irregular corneas
Contra-Indications
Allergy to any medications used during the procedure.
Materials
iCare disposable, sterilized tonometer probe
Setup
The iCare unit must be turned on and a probe loaded into it prior to measurement. To do so:
- Press the measurement button (the lower of the two buttons) to turn the unit on. The display will display “8.8:8.8”, then “on”, then “LoAd”.
- Load a new sterile tonometer probe into the unit, making sure that the probe base remains in a level or upright position to prevent the probe from falling out. Press the measurement button again. A “00” in the display indicates that the instrument is ready to measure.
- Position patient in an upright position and have them fixate straight ahead.
- Place the forehead rest on the patient’s forehead such that the probe is approximately 4-8 mm from the corneal surface, lined up with the visual axis. Adjust the forehead rest in or out as necessary.
- Take 6 individual measurements by repeatedly pressing the measurement button. A beep will be heard after each successful measurement. After six (6) successful measurements have been obtained, the IOP (in mm Hg) will be displayed after the letter “P”
- When finished, turn the unit off by pressing the selector button (the upper of the two buttons) repeatedly until the display reads “End”. Then press the measurement button for two seconds. The display will read “Bye” and the unit will then turn off.
- Dispose of used probe.
Results
If the “P” is not blinking, the measurements are considered accurate.
If the “P” is blinking, the standard deviation (SD) of the six measurements is greater than normal. Interpret as follows:
- If “P-“ (line up) is displayed, the SD is great enough to require re-measurement in all cases.
- If “P-“ (line in the middle) is displayed, the SD is greater than normal, but likely irrelevant. Re-measure if the IOP is over 19 mm Hg.
- If “P-“ (line down) is displayed, the SD is greater than normal, but not enough to affect relevance of the measurement. No re-measurement is required.
