34 Changes in Student Address or Name
Student address and name changes must be submitted as soon as possible. Name changes require legal documentation for the University and the ISDH.
Send all changes to the following places:
- Radiologic and Imaging Sciences Program Office at RadSci@iu.edu
- University (submit this change through One.iu.edu)
- Indiana State Department of Health (send e-mail to MedicalRadiology@isdh.in.gov)
