83 NMT Clinical Required Documentation
Required Documentation and Student Health Portal *
Students accepted into the Nuclear Medicine program should immediately set up a student patient portal with the Campus Health Office. Once set up, students can upload immunizations and other health records into the portal for verification and viewing by Campus Health staff. Students can also view his/her records at any time and print a copies if needed. Health records are less likely to get lost and the student is less likely to be on the Campus Health noncompliance list each semester when documents are stored in this portal.
Throughout the Program, students will need to supply current immunizations to Clinical Affiliates as they request them. Documents stored in the portal can be easily accessed and forwarded to requesting individuals. A quick reply to those requesting documents will ensure that the student will be able to remain in the clinic avoiding any unexpected absences.
Delays in providing documentation could prohibit the student from attending an assigned location. This would cause the student to use their personal time until appropriate documentation is supplied.
Required Immunizations for Clinical Rotations *
Students entering into the Nuclear Medicine Program are required to document compliance in a physical health screening, background check, drug screen, TB test, flu shot, and numerous other immunizations (HepB, Meningitis, MMR, Tdap, Td, Varicella) annually. In addition, all incoming radiography students are required to complete FIT Testing through Student Health.
The Health Professions Program (HPP), in conjunction with the Student Campus Health office, documents student compliance with all of these policies. The HPP office can address any questions about paperwork or compliance with this policy. The HPP office is located on the IUPUI campus in the Van-Nuys Medical Science Building at 635 Barnhill Drive, Room 203, Indianapolis, IN 46202. You may contact them at 317-278-4752 or at https://medicine.iu.edu/education/undergraduate-health-professions /contacts/.
Due to the length of the immunization process for Hepatitis B, a student may attend clinic in the fall semester (if at least 1 dose out of 3 has been given) but must show proof of completion of the Hepatitis B series by the beginning of the spring semester. At the end of the spring semester of the first year, students will be required to meet the same compliance issues for the second year of the program.
Students entering the Nuclear Medicine programs are required to document their immunization history and provide proof of health insurance. Students who do not comply with these requirements are ineligible for clinic activities and are subject to being check-listed with the university.
Required Health Insurance for Clinical Rotations*
Students entering the Nuclear Medicine programs are required to provide proof of health insurance. Students who do not provide health insurance documentation will not be permitted in clinic and are subject to being check-listed with the university. Information on health insurance plans for those who do not have insurance can be found at the following web-site https://health.iupui.edu/.
Required CPR Certification for Clinical Rotations*
Students are required to provide proof of CPR certification. The certification must be BLS for Health Providers. This information is collected when the student begins the program in the summer. The student is responsible for completing any necessary forms and/or attaching appropriate documentation in Canvas. Students must maintain CPR certification throughout the duration of the program. It is the student’s responsibility to renew their certification before it expires. No student will be allowed to attend clinic without proof of CPR certification. The student will need to take personal time off until CPR training is completed and a valid CPR card with an unexpired date can be presented to program officials.
Malpractice Insurance
Indiana University presently carries an umbrella, malpractice, insurance policy for all students enrolled in health related programs. This policy is in effect only while the student is engaged in activities that are part of the student program. It does not cover the student while involved in activities that are non-student related (such as part time employment).
