Clinical Affiliation Agreements
The IU Radiologic and Imaging Science Programs maintain Clinical Education Program Affiliation Agreements for radiography clinical experiences at several area hospitals and clinics.  These hospitals include:

The hospital Affiliation Agreements establish guidelines for student activity at the clinical sites.  Clinical sites have the right to ask the program to remove a student from the clinical site for any reason if they feel the student poses a risk to the health and welfare of the patients, staff, or the clinical site itself.  All Affiliation Agreements state that students must be in full compliance with IUPUI immunizations, health records, physicals, drug screens, and background checks.  Those students not in 100% compliance with these guidelines will be unable to attend clinic at these facilities.

A copy of each Clinical Education Program Affiliation Agreement is located in the shared drive and can be requested by contacting the program director.  Any questions regarding the terms of the agreement should be addressed to the Program Director.  The Radiologic Sciences clinical faculty member assigned to each clinical site is responsible for making sure that students understand the rules and policies for their particular institution.

Besides Evalue, this appendix will also list pertinent information about clinical sites.  Please review this information prior to your first day at a new facility. You will also find information about locating your clinical instructor on the first day.
